Employment Verification

CareeersUSA requires a $15.00 processing fee to complete all non-governmental employment verifications. CareersUSA's policy is to verify only dates of employment, title and ending salary.

To obtain employment verification by credit card:

1. Complete and submit our Employment Verification Request Form ("Employee Information" and "Company Requesting Information" sections only).
   Adobe® Acrobat Reader 8.0 or higher recommended.

2. Fax (561-995-7003) or e-mail a signed release from the employee authorizing CareersUSA to release employment information.

3. Click "Buy Now".

To obtain employment verification by check:

1. Complete our Employment Verification Request Form ("Employee Information" and "Company Requesting Information" sections only).

2. Obtain a signed release from the employee authorizing CareersUSA to release employment information,

3. Include a $15.00 processing fee (check made payable to CareersUSA), and

4. Send documents and check in a self-addressed envelope, to:
